Qualified Professional (QP/AP) - TMS Transitional Management Services


We are seeking a full-time Associate/Qualified Professional (AP/QP) for our Transitional Management Services Team. Working in Wake, Durham, Johnston, and Orange Counties, the AP/QP provides direct services to identified clients that are appropriate for TMS.

About SouthLight Healthcare

Founded in 1970, SouthLight Healthcare is one of the area's largest and long-established nonprofit providers of substance use treatment and mental health services. With outpatient and community-based programs, SouthLight provides prevention, education and treatment services in the Triangle and beyond. SouthLight has multiple office locations throughout the Triangle area, serving more than 9,000 clients each year. Services include adult outpatient counseling, Intensive Outpatient Program, Psychiatric Services and Medication Management, Opioid Treatment Program, Court Deferral Program, Peer Support Services, Family Centered Treatment, Community Support Team and a residential treatment program for women and children and more.


JOB RESPONSIBILITIES

  • Provide appropriate interventions to the TMS caseload under the supervision of the TMS Lead
  • Provide case management services as necessary to clients on caseload
  • Provide assertive engagement to appropriate clients that may engage in TMS
  • Meet with clients in the community, homes, and offices as needed (remain within the location ratio specified in service definition)
  • Assist with beneficiary housing search including engaging landlords to rent to beneficiaries and writing reasonable accommodation letters

QUALIFICATIONS


Educational and Experience

  • Two years' experience working directly with adults struggling with mental health and substance use disorders.
  • Two years of experience working in community mental health setting preferred
  • Provisionally Licensed MH/SA (LCASA, LCSWA, LCMHCA) or CSAC preferred
  • Must meet qualifications for AP/QP status

  • Excellent organization and time management skills
  • Maintain flexible and coachable demeanor
  • Ability to develop rapport with clients and other professionals in variety of settings
  • Ability and skills to set firm boundaries with clients
  • Excellent clinical skills including: evidence based practices, diagnostic information (DSM 5), substance use intervention, etc.
  • High degree of computer proficiency including but not limited to: Microsoft Office Suite, Outlook, and electronic medical records.
  • Excellent interpersonal skills and ability to work collaboratively within a team.
  • Comfortable working independently in the community
  • Excellent written/ documentation skills.
  • Ability to manage crisis situations both in person and over the phone

BENEFITS

Full-time employees are eligible for medical benefits the first of the month 30 days after hire date. Options for a standard plan and an HSA plan with a $300 initial contribution and up to a $25 per paycheck match. 10 paid holidays and PTO hours which accrue at a rate of 10 hours per month. After 6 months employees are eligible to enroll in our 401(k) plan which has a 4% match. Company paid life insurance, short and long term disability, in house and paid training opportunities, and additional pay for working holidays.
